Family Insurance Solutions Inc.
offer it’s employee’s a comprehensive benefit package that
includes: Extended Health Care, Dental Care, Vision Plan, a
Computer Purchase program and more. Family
covers 60% of the monthly premium for all private medical
coverages (Extended Health, Dental, Vision, Life, AD&D and
LTD).

Extended Health Care
|
|
Eligible costs are covered at
100%, there is a $25.00 deductible per year per person. This
includes Prescription Drugs.
There is $500.00 annual maximum for Professional
Practitioners including: Physiotherapist, Chiropractor,
Massage Practitioner, and Psychologist etc.

|
Dental Care
|
|
No deductible, basic
preventative and minor restorative services are covered at
100% with an unlimited maximum. Major restorative services
including: dentures, crowns and related services are covered
at 60% with a $2000.00 calendar year maximum. Orthodontia
for dependent children is covered at 50% to a lifetime
maximum of $1500.00.

Vision Care
|
|
No deductible. Eye exams are
covered to a maximum of $75.00 per person per 12-month
period. Prescription glasses are covered up to $250.00 every
24 months.

|
Life Insurance
|
|
200% of annual base salary up
to a maximum of $200,000 reducing by 50% at age 65.
Dependent Life Insurance: Spouse: $10,000.00
Child: $5,000.00

Accidental Death &
Dismemberment
|
|
Same schedule as above (Life
Insurance)
|
|
Long-Term Disability
|
|
Premiums are paid by the
employee and therefore non-taxable. 66 2/3% of the first
$3,000.00 of basic monthly income plus 45% of the remainder.
There is a maximum monthly benefit of $6,000.00 and a
non-evidence maximum of $4,500.00.

BC Medical Services Plan (MSP)
|
|
The company will pay 100% of
employee’s medical service plan premiums.
|

Group Retirement Savings Plan
|
|
After completing one year of
employment employees are eligible to participate in our
Group Retirement Savings Plan with Great West London Life.
Family Insurance Solutions Inc. will match employee
contributions up to a maximum of 5% of employee’s annual
base salary. Voluntary contributions above 5% are
acceptable.

Fitness Program
|

A healthy body leads to a
healthy mind and a better quality of living. We offer
a fitness reimbursement program where you are offered an
allowance of $300 annually towards a variety of fitness
related expenses such as gym memberships and court fees.

Employer Pass Program
|
|
Employees who use the transit
system regularly can take advantage of our employer pass
program. You will save money by being issued a Transit pass
through our employer pass program. The fees for the pass are
automatically deducted from your pay cheque.
